Vaccinations for Students in Kindergarten to Grade 8
Appointments are available from Region of Waterloo Public Health for elementary students who need to catch up on their routine vaccines. These routine vaccinations are required by the Immunization of School Pupils Act (ISPA).

Before you make your appointment
Before booking an appointment, families, caregivers or students should update their immunization records. It is important to ensure that all vaccinations your child received from your healthcare provider or out of province have been reported to Public Health.

A student’s Health Card number is needed to view their immunization record. If you do not have a health card, you can use the Ontario Immunization Identification (OIID) usually found on a child’s report card.
